Critical actions

Indication and risk named. Roles assigned: airway, meds, RT, monitor/recorder. Meds and doses confirmed out loud. Plan A, Plan B, Plan C stated. Post-intubation sedation and vent plan ready.

Pearls

A timeout should be short enough to actually use under pressure.
The phrase 'what changes after failure?' prevents repeated identical attempts.

Pitfalls

Skipping the medication double-check.
Not preparing post-tube sedation before paralysis.
